Coordinating patient care with other healthcare professionals
Treating behavioral health and medical conditions often requires the expertise of multiple treatment providers. To ensure patient safety as well as optimal treatment outcomes, it is important that these treatment providers share relevant and timely information regarding the patient’s health condition(s); treatment interventions, including medications; and treatment preferences. Additional treatment interventions may include:
- Management of treatment access and follow-up for members with coexisting medical and behavioral disorders
- Primary or secondary preventative behavioral healthcare program implementation
