Lucet has reached a significant milestone in transforming health care with the launch of Lucet™ at Home, a new product suite designed to help individuals manage complex chronic conditions with comprehensive in-home evaluations that address medical and behavioral health needs. This initiative reflects Lucet’s continued commitment to whole-person care by streamlining delivery and enhancing the high-risk member experience. Approximately two-thirds of Medicare enrollees have multiple chronic conditions which require coordination of care among primary care doctors, medical and mental health specialists, inpatient and outpatient facilities and ancillary services. Plus, nearly one in four U.S. adults have a mental illness each year, which increase the risk of chronic conditions and vice versa.
Individuals managing complex chronic conditions have long encountered a fragmented health care system, often resulting in missed opportunities for early intervention, avoidable hospitalizations and declining quality of life. Traditional care models frequently fail to identify risk early, close critical care gaps or intervene when symptoms arise. Barriers such as transportation issues and physical limitations often prevent high-risk individuals from accessing timely care.
On top of that, health plans see patients fall through the cracks, putting care quality and member experience at risk. Our system has long needed to address the gaps created through care fragmentation and poor coordination to drive better Healthcare Effectiveness Data and Information Set (HEDIS) performance measures or CMS Star Ratings.
Lucet at Home helps to address these challenges with a solution that brings whole health, patient-centered care directly into members’ homes. Licensed providers conduct comprehensive in-home assessments to evaluate health needs and guide care decisions. They also deliver coordinated, longitudinal care designed to reduce hospital visits and improve outcomes. This offering supports a whole-person approach to the full spectrum of physical, behavioral and social needs through integrated in-home medical care. It is particularly effective in identifying undiagnosed conditions, unmet behavioral health needs and social determinants of health that could otherwise escalate into higher costs and poorer health outcomes.
The Lucet at Home model shows significant clinical and financial impact. Health plans benefit from improved care quality, enhanced member satisfaction, and reduced total cost of care through preventive strategies. Outcomes include a 26 percent reduction in hospital admissions and per-member-per-month savings reaching $1,403 for certain conditions. Member satisfaction is high, with 91 percent indicating that their care team makes it easy to access necessary services. Additionally, this approach contributes to stronger HEDIS performance, improves CMS Stars ratings and supports health equity through integrated screening for social determinants of health.
